Methods of payment are Visa, Master Card, wire transfer, bank draft or cheque made payable to Underwriters' Laboratories of Canada.

The order form is available Online. You can either use the link below or contact us and we can send one to you.

If you are ordering labels for the first time you will need to work with our group to compose a label design that encompasses all the required components. Please fax or email us a proposed design or ask us to help you with your proposal.
